*All participants will be required to follow MN Covid guidelines or Ortonville School Districts/MNSHL Covid guidelines

*All participants must sign and return the Covid 19 Waiver prior to participate*
One form may be filled out per family. If you need additional room, you may use the back of this form or use another registration.

Please fill it out completely; if there are discrepancies, forms will not be processed.

Payment:  Please attach payment to the form(s) and submit either by mail or dropping it off at the City Office.
